Process

CenterPath Wellness is a provider-recommended mental health service, and we welcome referrals from all sectors. Our licensed clinicians benefit from decades of experience working with a diverse range of populations and mental health disorders. We provide a safe, reliable landing place for your patients to pursue their goals and thrive. We even offer telehealth services for select programs.

To refer a patient to receive quality mental health services, please fill out the following referral form and scan or email the document to Referrals@MyCenterPath.org or fax us at 908-756-5566.

If you have any trouble reaching us or want to learn more, you can always contact the front desk and speak with one of our knowledgeable staff, who will help you navigate the process or answer any questions you may have.

We are a Medicaid-approved mental health treatment center, also accepting Medicare and most private insurances but also find opportunities to serve those without insurance. Our team will help them navigate what is available to them and help them find the help they need.

CPW Intake Form

Use this form if you’re a prospective client/hospital/agency interested in services at CenterPath Wellness. Please complete the Referral and fax it to our office at 908-756-5566. Our Administrative Coordinator will contact you once received to conduct the pre-intake. Then, a Clinical staff will contact you to get you set-up for services!
