What Comes Next
You will then be scheduled for an intake with a clinician, which will take approximately one hour. Some questions you may be asked:
1. What is the current problem/issue or diagnosis?
2. Are you currently using illicit drugs or alcohol?
3. What services are you looking for?
4. Do you have any current thoughts of hurting yourself or others?
5. Do you have any allergies?
Based on your appointment, the clinician may schedule another appointment to ask you more in-depth questions about the issues that brought you to the agency for services, as well as information about your history.
Depending on your needs and the severity of your diagnosis, you will be placed in one or multiple of our programs (Individual Therapy, Medication Management or Partial Care). Please note: Everyone will receive at least one psychiatric evaluation at the beginning of treatment to be evaluated by a medical practitioner; however, no one is required to take medication.
You will either be assigned a therapist or a primary PC counselor, who will be responsible for providing or managing your wellness journey. Please note: If you are receiving Medication Management only, you will be assigned an APN for medication treatment.
From there, our committed and trusted staff members will work with you on the best path for you and will work with you on your journey to your wellness goals!
Insurance and Payment Options:
CenterPath Wellness is a Medicaid-approved mental health treatment center, also accepting Medicare and most private insurance plans. We also provide services to individuals without insurance. If you are currently uninsured, you may be eligible for state-paid services if you meet mandated state income criteria.
Please note that all co-pays, co-insurance, deductibles, and outstanding balances are due at the time of appointment. Thank You.
Cancellation Policy
Although we understand that sometimes things come up, it is very important to keep your appointments. If you need to reschedule, please do so 24 hours in advance. Late cancellations will be handled at your provider’s discretion.
An individual who does not attend or cancel their appointment is taking time away from another client. Because of this, we have a no-show policy:
If you miss one appointment without calling, we will contact you by phone.
If you miss two appointments without calling, we will send you a letter.
If we do not hear from you within two weeks of the letter or you miss a third appointment without calling to cancel, you will be discharged from services.
